HEALTH HISTORY QUESTIONNAIRE INSTRUCTIONS

As per the 7^th Battalion, members of the 21^st Georgia must now carry with them Medical Information onto the battlefield at each event.  The 21^st has since mandated the maintenance of Medical Information by all members at every event.

 

     1)     Save the 21^st Georgia Health History Questionnaire to your computer.

     2)     Enter all information requested on the form.

     3)     Upon form completion, save the form and e-mail it to the Civilian Coordinator at 

             Civilian@21GA.org.

 

     4)     When additions or modifications need to be made, edit the form that has been saved to your

             computer, and again e-mail the updated form to the Civilian Coordinator.

 

Minors who will not be with their parent or legal guardian must also have a valid Minor Medical Release Form completed.

 

If additional information needs to be added to the form, but space is unavailable on the form, please include it in the e-mail to the Civilian Coordinator.
